Implementation of the “Noncommunicable diseases (NCDs) outreach mobile teams” project in Lviv region

The project  “Noncommunicable diseases outreach mobile teams”  is governed by the  Charitable Organization Charity Foundation “WOMAN HEALTH & FAMILY PLANNING”. The project is implemented in partnership with  the Ministry of Health of Ukraine, Public Health Center MOH of Ukraine, Regional Health Departments. Activities are performed in 4 regions of Ukraine – Lviv, Poltava, Rivne and  Dnipro.

The aim of the  project is improving access to primary health care services with a focus on NCDs  and to provide access of patients to free of charge medicines. This aim is reached by means of activity of multidisciplinary mobile teams  reaching vulnerable population in hard to reach areas. Each mobile team consists of  family medicine doctor (if necessary: specialist, e.g. endocrinologist, neurologist, etc.), nurse, psychological support specialist, driver, volunteer / community health professional. Each mobile unit should perform not less than 20 visits per month to small cities and villages and to provide services not less than to 20 persons during each visit.

During the first month of the project activities the Lviv multidisciplinary mobile team has developed    19 visits to 16 small cities and villages and has provided services close to 400 persons. Implementation of the project is of great success.
